ABOUT US

At Life Reimagined, we firmly believe that mental health treatment should never involve medication alone. Restoring our mental well-being is more complex than simply taking a pill. Each person and the life experiences that came along are different. Optimal treatment consists of developing a solid relationship with a provider who will take the time to get to know you. For this reason, initial visits last approximately 60 minutes and follow-ups 30 minutes. Each visit is used to develop a treatment plan that suits your individual needs. Psychotherapy is incorporated into each session with medication management. Space is provided for patients to be their authentic selves, safe, and active participants in their mental health care.

Tashonna Webster, PhD, MPH, MS, PMHNP-BC

Dr. Webster is a board-certified psychiatric mental health nurse practitioner. She has a doctoral degree in public health, which sparked her desire to help individuals improve their health and well-being. Dr. Webster’s knowledge of mental and public health means she has a solid foundation for diagnosing and treating illnesses and understanding how psychosocial factors at the individual and community level contribute to her patient's health status.

Adult Psychiatry 

Phone Consultation

 

This is a brief conversation, no more than 15 minutes., to determine if there is a good fit between the patient and provider.  This is an opportunity to ask questions about the practice and determine if you would like to set up an initial evaluation.

Follow-up Extended (Medication Management + Psychotherapy: 30 minutes)

 

This is a combined session that focuses on medication management and psychotherapy.  Medication management will focus on whether the achieved outcome is achieved as well as side effects and patient education.  Psychotherapy will also be conducted to address any patient concerns or life stressors. 

Initial Psychiatric Evaluation (45-60 minutes)

An initial evaluation is used to help develop and individual treatment plan.  During the session, information is gathered about past medical psychiatric, and family history. We will discuss your overall functioning, quality of life, and whether you've engaged in substance use.  We will assess your mood, anxiety, cognition, behaviors, thought processes, and perceptions.  Gathering all this information will help better assess your psychiatric needs and develop a comprehensive treatment plan 

Follow-up (Medication Management: 15 minutes)

This appointment will focus on medication alone in terms of side effects and dose adjustments. 

 

Life Reimagined is a telehealth-only based practice, so in-person appointments are not offered at this time. This has important considerations for prescribing controlled substances. Controlled substances include medications such as, stimulants (Adderall, Ritalin, Vyvanse, etc.); benzodiazepines (Klonopin, Xanax, Ativan, etc.); sedative-hypnotics (Ambien, Sonata, Lunesta, etc). Please discuss this during your phone consultation if you anticipate requiring these medications.

Cancellation

No Show

We reserve your appointment time to offer you our full attention and care. If you need to postpone your appointment, you are required to give at least 2 full business days notice (48 hours NOT including holidays and weekends). This allows us to offer that appointment slot to another patient in need. Cancellations within two business of the appointment as well as any missed appointments will be charged the full appointment fees. Please note your insurance will not cover a missed appointment.

Late Attendance

If you are more than 15 minutes late for your appointment, we will not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our best to reschedule your service for another time that is convenient to you.

Credit Card on File

All patients must have an active credit card on file in order to receive treatment at Life Reimagined. Credit card information will be obtained upon scheduling your first appointment.

Form Completion Letter Writing

We understand that various forms or letters may be required to assist you with your healthcare needs. Our providers will be happy to complete forms and write medical letters as necessary upon your request.

A service fee of $25.00 will be charged for simple forms. For complex or rush-order form completions, the fee is $40.00.

Prior Authorizations

Prior Authorizations may be requested by insurance companies to allow for your medications, or certain services, to be covered. While we continue to urge health insurance companies to abandon this practice, our office must cover the additional administrative costs associated with the process of completing prior authorization requests by charging a $25 fee to the patient. 

Prescription Refills

Refills should be requested at regularly scheduled visits. Medication adjustments and changes can only be made during scheduled visits, unless there are medication side effects that necessitate changes before a visit can be scheduled.  Refills will only be sent when there is a scheduled upcoming appointment.

Telephone Calls

Phone calls made during normal business hours will be returned within two business days.  For emergencies, call 911 or go to the nearest emergency room.

Calls lasting longer than 10 minutes are subject to a $200 visit fee. If you anticipate needing at least 10 minutes for your call, or would like to discuss a non-urgent change to your treatment (including dose changes), please instead schedule a telehealth appointment so that we may best address your concerns.

Privacy Policy

We are committed to protecting your privacy and keeping your personal health information confidential. Our practice collects personal information, including name, phone number, email address, medical history, treatment records, and billing information, through our electronic medical records system and contact form to provide you with quality psychiatric care and process insurance claims.

 

We store this information securely, complying with all applicable healthcare privacy laws and regulations, including HIPAA. We do not sell, rent, trade, or disclose your personal information to third parties for marketing purposes. We may share information with payment processors, legal authorities, and care partners only as required to perform essential healthcare and administrative functions, or as required by law, and always with your explicit written consent where applicable.

 

Our electronic medical records system employs industry-standard security measures to protect your data, and access to your information is strictly limited to authorized healthcare providers and staff members directly involved in your care. We maintain detailed records of all access to your information and regularly review our privacy practices to ensure the highest protection for your personal health information.

 

By providing your phone number or submitting a contact form through our website, you consent to receive SMS text messages from our office for the purpose of coordinating care, including scheduling a brief phone consultation, medication requests, and communication with staff at senior living facilities. Message and data rates may apply. Message frequency may vary. You may opt out of SMS communication at any time by replying STOP to a message or by contacting our office. You can also text HELP to receive assistance.

 

We will not share your SMS consent or phone number with any third party or affiliate for marketing or promotional purposes. All SMS communication practices comply with CTIA messaging guidelines (section 5.2.1).

 

If at any time you would like your information removed, you may contact our office by phone or through our website.

  4. Privacy and Confidentiality: All messages will comply with HIPAA guidelines to ensure patient privacy and data security. Do not include sensitive patient details unless explicitly necessary and permitted.

  5. Message Rates: Standard message and data rates may apply based on your mobile carrier’s terms and conditions.

  6. Opt-Out: To opt out of SMS communications, reply "STOP" at any time. Note that opting out may limit our ability to provide timely updates related to patient care.

  7. Support and Assistance: For questions or assistance, reply "HELP" or contact our office at 516-229-1394 during business hours.
